Job Description
Location: Charlotte, NC
We are seeking a dedicated and organized Case Manager to join our client’s team. This is a remote role; however, candidates must reside near Charlotte, NC, to attend occasional in-person meetings or client visits as needed. The Case Manager will be responsible for coordinating and overseeing services to ensure clients receive the support they need to achieve their goals. This role involves working closely with clients, families, and external providers to develop and implement effective care plans. The ideal candidate will have strong interpersonal skills, attention to detail, and a commitment to providing excellent client service.
Responsibilities:
- Conduct assessments to identify client needs and develop personalized care plans.
- Coordinate services and resources to meet clients’ individual goals and needs.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
- Advocate for clients to ensure they have access to necessary resources and services.
- Collaborate with healthcare providers, social workers, and community organizations.
- Maintain detailed and accurate records of client interactions and progress.
- Ensure compliance with all relevant policies, procedures, and regulations.
- Provide ongoing support and guidance to clients and their families.
- Conduct follow-ups to assess client satisfaction and outcomes.
- Participate in team meetings and case conferences to ensure coordinated care.
Qualifications:
- Bachelor’s degree in social work, psychology, or a related field required.
- Active Case Manager Certification (CCM) or eligibility to obtain certification preferred.
- Minimum of 2 years of experience in case management or a related role.
- Strong understanding of community resources and support systems.
- Excellent communication and organizational skills.
- Ability to work independently and manage a caseload effectively.
- Proficiency in using electronic case management systems and Microsoft Office.
- Strong problem-solving and critical-thinking skills.
- Commitment to maintaining client confidentiality and ethical standards.
- Must reside near Charlotte, NC, and have the ability to attend occasional in-person meetings or client visits.
Salary Range: The salary range for this position is $60,000 – $75,000 ($28.00 – $36.00) a year based on experience.
